What to Bring With You

For Your Hospital Stay

For a hospital stay, we urge you to limit personal belongings. Valuables should be sent home or given to a nurse for safekeeping. Follow your care team's instructions for specific items to bring with you for your stay.

For Your Clinic Visit

Your clinic may provide a list of items to bring for an appointment in addition to those below. Please bring:

  • Name and address of your referring doctor and primary care doctor
  • Contact information for any other provider you would like us to contact about your plan of care
  • List of any medicines (and dosages) that you are or have been taking (including over-the-counter and herbal medicines)
  • List of any known food or drug allergies
  • Important past medical information. If you have been asked to bring it, hand-carry a disk with any x-rays or other imaging results, even if you’ve asked that your clinic send them
  • Social security number
  • Your health insurance card and any required authorization or referral forms
  • Your co-payment or payment due at the time of service (We accept cash, personal check, Visa, MasterCard, American Express and Discover.)
  • A list of questions to ask your doctor to be sure we’re addressing all of your concerns
