Social workers help patients and families face the challenges that illness can bring. When a family member or friend faces serious illness, many wonder where to turn for support and answers. We provide clinical social workers who work directly with patients, caregivers and families to address the many aspects of a person’s life that are affected by illness or medical care. Through in-depth assessments, problem-solving, and collaboration with the interdisciplinary team, we work with each person as an individual to support their needs.
Social workers will:
- Develop a holistic care plan to address the unique circumstances or concerns that each patient may face
- Provide emotional support and adjustment to illness counseling for patients and their families
- Assist with transition planning back into the home or community to meet patients’ healthcare needs
- Access community-based resources for ongoing medical care, financial assistance, and counseling support outside the hospital or clinic
