Symptom Management Log (pdf)
Not all patients experience side effects from their treatment, and how you react to each treatment varies for each individual. For those that do experience side effects, many patients find it helpful to keep a log of what they are experiencing. This log can be used at your next medical appointment to provide helpful information to your providers.
Medication Tracking Sheet (pdf)
It is important to track your current medications, including over-the-counter drugs, and keep your care team informed.
Fluid Intake Tracker (pdf)
During most cancer treatments, it is essential that you drink enough liquid to help your body heal from treatments. We recommend drinking 8-10 cups of liquid (includes thickened supplements) every day. Aim to drink 1/2 cup per hour during the day to achieve the recommended amount. If you are using a nutritional supplement, please keep track of the number of servings you drink per day. This tracker will help you monitor your daily intake.
